This position is an onsite position located at the company headquarters. The Patient Success Manager will supervise all staff and activities related to patient enrollment and recovery. The Patient Success Manager will be responsible for monitoring, analyzing, and driving patient enrollment, engagement, and retention. The Patient Success Manager will assist with the development of action-oriented policies and strategies for patient outreach programs designed to meet the needs of our client and the goals of NavCare.

Essential Job Functions/Responsibilities:

  • Understands company goals and expected performance metrics as they relate to enrollment and retention.
  • Understands the benefit of Chronic Care Management (CCM), Remote Patient Monitoring (RPM) and Transitional Care Management (TCM) and can articulate the benefits of the programs to internal and external stakeholders.
  • Responsible for implementation of outreach programs to team.
  • Works collaboratively with Client Success Managers to understand client expectations as they relate to enrollment optimization and patient retention.
  • Provides leadership, coaching and training to the enrollment team.
  • Follows all privacy policies and maintains the confidentiality of protected healthcare information (PHI).
  • Ability to learn and keep up to date with proprietary software.

    Position Qualifications:
  • Ability to quickly demonstrate expertise and establish credibility.
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred.
  • Two (2) years of management experience in a healthcare setting.
  • Experience with a variety electronic health records or electronic medical records systems and the ability to quickly master new IT systems.
  • Exceptional customer relationship building skills, along with strong data entry and verbal/written communication skills, are essential.
  • Proficient in Microsoft Office applications (Excel, Power Point, Word, etc.).
  • High degree of flexibility and organization.
  • Ability to multi-task, meet deadlines and work in a fast-paced environment.
  • Ability to build and develop a high functioning team.
