an advanced level practical nurse position. The Charge Nurse works
in collaboration with the Nurse Manager, Director of Nursing,
Program Director, Unit/Floor Manager and physician(s) to plan,
direct, and evaluate the care provided for clients and their
families. The Charge Nurse possesses expert clinical knowledge,
which the Charge Nurse applies to collect and assess data in
collaboration with the multidisciplinary team to provide
therapeutic care systematically and continuously.The Charge Nurse
communicates routinely with the physician(s) and the direct care
leadership and/or other health team members to establish goals and
strategies for meeting the continuing care needs of the client,
family, and/or other care provider. care in accordance with nurse
practice acts and the professional nursing role.The Charge Nurse
maintains expert clinical knowledge in nursing practice and assumes
responsibility for the professional development of self, other
direct care staff, and students. The Charge Nurse serves as a role
model and clinical resource for other nursing personnel and direct
care team members.What you bring to the table:EducationDegree from
an accredited school of nursing; is a basic requirement for all
Registered Nurses.ExperienceTwo (2) years - Four (4) years of
experience as an RN, such as could have been attained in a long
term care, med/surg or inpatient psychiatry, or similar setting
that has provided the knowledge, skills and abilities necessary to
perform satisfactorily the duties of the position may be
considered.RN at this level are expected to perform all duties
within the scope of their license (e.g., medication administration,
floor nurse assignments). However, completed work may require
routine or detailed higher-level review depending upon the
complexity of the duties involved.Two (2) years of increasingly
responsible leadership experience to include charge, lead or
supervisory responsibilities. (Preferred)Two (2) years of inpatient
psychiatry experience (Preferred)Licenses/CertificationsMust
possess a current, unrestricted California license as a RN
(required)Current CPR/BLS certification (required)CPI Certification
(provided by the facility within first 90 days of employment).What
you will do:KEY JOB RESPONSIBILITIES:Initiates multidisciplinary
collaboration to positively impact the outcomes of care provided.
Effectively communicates with the medical staff, clinical team,
Nursing and Program Leadership to solve complex multidisciplinary
problems.Communicates with referring facilities and emergency
medical services regarding clients expected in the
facility.Coordinates the flow of client admissions, transfers, and
discharges with other Charge Nurses as needed. Ensures distribution
of workload among team members and determines assignments based on
skills and abilities of the team through effective delegation and
supervision.Coordinates the reception and delivery of the team's
shift report.Initiates steps to support and enhance the client's
responsibility and self-determination in decision making concerning
health, treatment, and wellbeing. Involves the individual
client/family in identification of requirements/perceptions of
learning needs and preparation to assume self-care upon
discharge.Acts a leader through participation in unit and/or
departmental quality assurance structure in order to improve
care.Informs on coming charge nurse of residents' condition(s),
services rendered during the shift, and the plan of care which has
been developed.Participates in department and facility performance
improvement activities.Implements actions prescribed by DON that
are necessary to correct deviations from established standards of
documentation and nursing care (incident reports, plans of
correction, documentation deficiencies, etc.).Participates in
debriefing following an incident as necessary and facilitated by
staff leads.Performs other duties as assigned.Wellpath is an
