Job Description:
Agape Care Group is seeking a compassionate and skilled Nurse
Practitioner to join our team, providing top-tier hospice and
palliative care across the Southeast. As a vital member of our
healthcare team, you will play a crucial role in enhancing our
patients quality of life by delivering personalized and empathetic
care in accordance with our values of dignity and comfort. In this
role, you will work closely with physicians, patients, and their
families to ensure the effective delivery of our healthcare
services during lifes most significant moments. Conduct
comprehensive patient assessments to gather information about their
conditions and the symptoms they experience. Develop and implement
patient management plans, record progress notes, and assist in
provision of continuity of care. Prescribe medication in accordance
with state and federal regulations. Collaborate with physicians,
other nurse practitioners, and healthcare professionals to
formulate a comprehensive care approach. Guide patients and their
families through the palliative care process, providing clear
communication and compassionate support. Monitor patient's
condition and adjusting treatment plans as necessary. Perform
clinical procedures such as advanced wound care, post-surgical
care, and bedside care. Educate patients and families on
end-of-life care plans, including symptom management and comfort
care strategies. Strong clinical skills with the ability to perform
and interpret diagnostic tests. Excellent communication and
interpersonal skills for interacting effectively with patients,
families, and health care providers. Proficiency in using medical
software for health records. Strong ethical mindset and the ability
to maintain the confidentiality of medical records. Ability to
handle emotional situations and provide a high level of patient
care. Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Current licensure as a Nurse Practitioner in the
state of practice. Certification in Palliative Care Nursing (CPN)
or willing to obtain within 1 year of employment. DEA registration
certification.
